Transitions strategy 2020-2024

The Transitions Strategy 2020-2024 (PDF) has been co-produced with young people, parents and carers and wider colleagues and partners such as BSW ICB and Adult Social Care.

The strategy is designed to be interactive with links to relevant pages to give more information on certain topics.

The strategy will be updated every six months to stay current along with reviewing the progress on the Strategy Action Plan with the project group and feeding back progress to the SEND Board and Transitions Working Group.

Transitions for children and young people with SEND

A virtual booklet, SW Transitions regional project 2023, has been produced following work carried out by the Regional Parent Carer Forums, and completed for NHS England South West on transitions.

It is intended for professionals and families, along with children and young people, when supporting those going through transition.
