The ‘My Care My Views’ form is used to find out what people think about their care and support
Filling in the form will help us to check that care and support for people in Swindon is as good as it can be.
It will help us to make sure people know about their choices about care and support.

If you want us contact you about this work we will need some of your personal information:
- Name
- Telephone number
- E-mail address
- Home address
Your personal information will be saved on a file on our computer system.
It will be kept safe and not shared with anybody else.
You don’t have to give us this information. It’s up to you.
- How the law allows us to use your information?

We will use your information to:
- contact you about the form you filled in
- we will only contact you if ticked the box to tell us you are happy for us to do this
