Sustainability transformation plan and NHS five year forward view

All health and social care organisations are working together across Bath and North East Somerset (B&NES), Swindon and Wiltshire, on a five year sustainability and transformation plan (STP), to improve our local population’s health and wellbeing, to improve service quality and to deliver financial stability.

We are developing a joint approach that will help deliver the aims of the Five Year Forward View and is in line with other important national guidance such as GP Forward View, Mental Health Taskforce Report and National Maternity Review.
